ผลการนำนโยบายการบริการสังคมแก่ผู้สูงอายุของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ไปสู่การปฏิบัติ

Abstract

The objectives of this quantitative study were to explore : 1) outcomes of the adoption of a policy on social services for the elderly of the community health center, Maejo Municipality and 2) factors affecting the implementation of the policy. The sample group consisted of the community health center staff, public health volunteers and the elderly in Maejo Municipal area. Obtained data were a analyzed by using inference statistics and descriptive statistics ie, frequency, mean, and standard deviation. Besides, multiple regression technique was employed in this study.

Results of the study revealed that, as a whole, outcomes of the adoption of policy on social services for the elderly was found at a high level of the agreement (xˉ = 3.94, S.D. = 0.42). This was on the basis of the following : care-taking (xˉ = 3.99, S.D. = 0.56); co-activity doing area (xˉ = 3.97, S.D. = 0.54); care-taking of aging patients in the bed (xˉ = 3.91, S.D. = 0.56); sanitary (xˉ = 3.91, S.D. = 0.55); and quality of life (xˉ = 3.93, S.D. = 0.53). Meanwhile, factors affecting the adoption of the policy were service provision, determination of mission and task designation, and community participation (0.339, 0.317, and 0.242, respectively).

The following were suggestions on the adoption of the policy : 1) The management of the community health center focused on 3 aspects : quality services – health problem screening, quality services for aging patients at home, and care-taking manager / care-takers. 2) Appropriate task designation – clear task designation, work rotation, and knowledge / skill development. 3) community participation – based the elderly networks, public health volunteers, exercising network, and local people venue. For the participation in operation, the community health center joined projects on home visit, public health volunteer, extension of academic knowledge and process to the community based on local people participation, etc.

การวิจัยเรื่องผลการนำนโยบายการบริการสังคมแก่ผู้สูงอายุของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ไปสู่การปฏิบัติ มีวัตถุประสงค์ 2 ข้อ คือ 1) เพื่อศึกษาผลการนำนโยบายการบริการสังคมแก่ผู้สูงอายุของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ไปสู่การปฏิบัติ 2) เพื่อศึกษาปัจจัยที่ส่งผลต่อการนำนโยบายการบริการสังคมแก่ผู้สูงอายุของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ไปสู่การปฏิบัติ

การวิจัยครั้งนี้ ใช้ระเบียบวิธีเชิงปริมาณ (Quantitative Research) ประชากรที่ใช้ในการศึกษาครั้ง คือ 1) เจ้าหน้าที่ของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ 2) กลุ่มอาสาสมัครสาธารณสุข 3) ผู้สูงอายุในเขตพื้นที่เทศบาลเมืองแม่โจ้ อำเภอสันทราย จังหวัดเชียงใหม่ รวมจำนวนกลุ่มตัวอย่างที่ใช้ในการศึกษาทั้งหมด 112 คน สถิติที่ใช้วิเคราะห์ คือ สถิติพรรณนาในการวิเคราะห์ข้อมูลทั่วไป ค่าความถี่ ค่าเฉลี่ย และค่าเบี่ยงเบนมาตรฐาน และสถิติอนุมาน (Inference Statistics) โดยเทคนิคการวิเคราะห์การถดถอยเชิงเส้นพหุ (Multiple Regression)

ผลการวิจัย พบว่า ผลการนำนโยบายการบริการสังคมแก่ผู้สูงอายุของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ไปสู่การปฏิบัติ โดยภาพรวม พบว่า ผลรวมอยู่ในระดับเห็นด้วยมาก (xˉ = 3.94, S.D. = 0.42) เรียงลำดับตามระดับความคิดเห็นเฉลี่ย ได้แก่ ด้านการดูแลเอาใจใส่ (xˉ = 3.99, S.D. = 0.56) ด้านพื้นที่ในการทำกิจกรรมร่วมกัน (xˉ = 3.97, S.D. = 0.54) ด้านการดูแลเอาใจใส่ผู้สูงอายุติดเตียง (xˉ = 3.91, S.D. = 0.56) ด้านสุขภาวะ (xˉ = 3.91, S.D. = 0.55) ด้านคุณภาพชีวิต (xˉ = 3.93, S.D. = 0.53) ส่วนปัจจัยที่ส่งผลต่อการนำนโยบายการบริการสังคมแก่ผู้สูงอายุของศูนย์สุขภาพชุมชนเทศบาลเมืองแม่โจ้ไปสู่การปฏิบัติ เรียงลำดับจากค่าอิทธิพลเชิงสาเหตุมากไปหาน้อย คือ การให้บริการ การกำหนดภารกิจและมอบหมายงาน และการมีส่วนร่วมของชุมชน โดยมีอิทธิพลเชิงสาเหตุเท่ากับ 0.339 , 0.317 และ 0.242 ตามลำดับ

ข้อเสนอแนะในการนำนโยบายไปสู่การปฏิบัติ คือ การบริหารศูนย์สุขภาพชุมชน มีจุดเน้น 3 ประการ คือ (1) เน้นการให้การบริการที่มีคุณภาพ กล่าวคือ 1) การมีระบบคัดกรองปัญหาสุขภาพ 2 ) มีบริการการดูแลสุขภาพผู้สูงอายุที่บ้านที่มีคุณภาพ (Home Health Care) 3) มีผู้จัดการการดูแลผู้สูงอายุ (Care manager) มีผู้ดูแลผู้สูงอายุ (Care giver) 4) มีระบบการดูแลผู้สูงอายุ กลุ่มติดบ้าน กลุ่มติดเตียง และมีแผนการดูแลผู้สูงอายุรายบุคคล (Care plan) 5) มีชมรมผู้สูงอายุผ่านเกณฑ์ชมรมผู้สูงอายุคุณภาพ (2) กำหนดภารกิจและมอบหมายที่เหมาะสม กล่าวคือ 1) มีการแบ่งงานอย่างชัดเจน 2) มีการหมุนเวียนการทำงาน และ 3) มีการพัฒนาความรู้และทักษะอย่างเพียงพอ และ(3) เน้นการมีส่วนร่วมของชุมชน กล่าวคือ จัดให้มีการมีส่วนร่วมระหว่างเครือข่าย ได้แก่ 1)เครือข่ายผู้สูงอายุ 2)อาสาสมัครสาธารณสุข(อสม.) 3)เครือข่ายการออกกำลังกาย 4)เวทีประชาชน โดยการมีส่วนร่วมในการปฏิบัติทางศูนย์สุขภาพชุมชนได้มีส่วนร่วมในการปฏิบัติงานร่วมกัน เช่น โครงการอาสาสมัครออกเยี่ยมบ้าน โครงการอาสาสมัครสาธารณสุข(อสม.) และการออกพื้นให้ความรู้และกระบวนการวิชาการให้กับชุมชน โดยเน้นกระบวนการมีส่วนรวมของประชน
